What is "Hospital to Home" support?
It’s a service that helps NDIS participants transition from hospital back to their home (or SDA/SIL), with support in medical, mobility, routine and environmental needs so recovery is safer and more comfortable.
You must be an NDIS participant planning to leave hospital (after surgery, injury, illness etc.), or transitioning into supported accommodation, or need short-term recovery / support at home.
Ideally before your hospital discharge or as soon as possible. We can begin pre-discharge planning while you’re still in hospital to ensure a smooth transition.
Medication management, mobility aids and equipment, personal care, daily living routines, home safety modifications, assistive technology, linkages to allied health and community services.
Yes — depending on your needs. We can arrange flexible or around-the-clock support if your condition or situation requires it.
We assess your home environment, work with therapists or occupational therapists, and recommend modifications or technology. We can help coordinate funding via your NDIS plan.
We collaborate with the hospital discharge team, therapists, your NDIS planner, and any relevant health professionals to ensure continuity of care and that your care plan reflects your medical and functional needs.
We monitor your progress and adjust the support plan accordingly — scaling up or down support as needed, modifying routines or equipment, or liaising with health professionals if required.
While no service can guarantee zero risk, our goal is to reduce risk of readmission by ensuring safe home environments, good medication management, appropriate supports and close monitoring.
